Use this original worksheet with the person making the transition and the supporters they choose. Ask the treating team to clarify clinical recommendations; use the worksheet to organize the practical arrangements. Print it and keep the completed copy in a private place.

Make each line concrete
“Therapy next week” needs a date, receiving provider and confirmation. “A relative will drive” needs the relative’s agreement and a travel time. If the arrangement is unresolved, write “unconfirmed” and name the person who will check. This keeps an intention from becoming an apparent commitment.

Decide how the review will work
Ask the person which updates they want to share with their supporters. Confirm the provider’s information-sharing process separately; paying for support does not define the complete sharing arrangement. HHS guidance.
At the review, ask what happened, which practical obstacle appeared and what the next step requires. Adjust the logistics with the person. Bring changes that need clinical judgment to the treating team.
