A named address leaves other questions open. Is arrival arranged? Can the person reach the appointments they have agreed to attend? Who has the relevant contact information? What practical change would require another conversation? Discuss these details with the person and the appropriate professionals rather than assuming that everyone means the same thing by “going home.”
A manageable weekly rhythm
Build the first schedule around confirmed commitments and the person’s priorities. Allow travel time and ordinary daily tasks. Keep it small enough to review together. If a plan is not being used, first identify the practical obstacle: timing, location, cost, unclear responsibility or a support arrangement that does not fit.
Family participation and privacy
